Aims of Social Work and Case Management
Social work case management aims to:
- Provide a comprehensive needs assessment in order to promote or maintain quality of life and the optimum level of functioning in the community
- Consult with you to develop a care plan
- Provide you and other service providers with a single point of contact by coordinating your health and wellbeing services
- Enhance your choices and actions in health and lifestyle matters by making information accessible and understandable
- Advocate for improvements in service provision, linking across services and informing policy making to enhance the service system
Aside from providing the above Social Work services, we can also refer you to other services if needed. A wide range of health services are available at BHN.
Our multidisciplinary approach helps us meet your needs as BHN provides a full range of health and wellbeing services. Our staff is highly qualified and experienced.
Accessing Social Work Services
Requirements for eligibility:
- Older adults who are frail and/or disabled
- Younger people with disabilities
- Chronic and complex condition patients, as well as their caregivers
Service hours
Our service is available from 9am – 5pm, Monday through Friday.

We offer Social Work services in your home or at our locations in:
- St Kilda
- Prahran
- South Melbourne
- Parkdale
- Bentleigh East
