Accessing the service
Eligibility requirements for this service are:
- For clients aged 65 years or over please contact My Aged Care
- For clients aged 25 – 65 please complete an online referral or contact intake/triage
When to access the service
Service hours are Monday through Friday, 9am – 5pm.
Service location
As we are a home visiting service, our clients must reside within the following areas:
- City of Kingston
- City of Bayside
- City of Glen Eira
- City of Port Phillip
- City of Stonnington
- City of Melbourne**We service parts of these areas
