Mental Health Integrated Complex Care

In Mental Health Integrated Complex Care (MHICC) services, client care is coordinated over an extended period of time to address the complex needs of people with severe mental illness.

About MHICC

Services provided by MHICC support clients who are experiencing severe mental illness and have complex needs requiring enhanced coordination of care for an extended period.

In order to meet an individual’s needs, four key elements of the service are designed to be highly flexible:

  • Counselling and mental health nursing services
  • Support and liaison with family
  • Liaison and coordination of care
  • Enhancing access to health care, including mental health services

Accessing BHN MHICC Services

People who can benefit from BHN MHICC services include:

  • Those living or working in Kingston, Stonnington, Bayside or Glen Eira
  • Those who have health care card or can’t afford similar services
  • Those ineligible for Partners in Recovery (PiR) or the National Disability Insurance Scheme (NDIS).

In order to qualify for MHICC services, people must also:

  • Have a serious mental illness affecting their social, personal or work lives; have complex needs; or be at risk of hospitalisation if not treated and cared for appropriately
  • Have a healthcare practitioner currently managing their care in a primary healthcare setting, such as a GP or psychiatrist
  • Have a GP Mental Health Treatment Plan, or an equivalent plan prepared by a psychiatrist or appropriate healthcare professional.

Being referred to a MHICC service

People can refer themselves or seek the help of their general practitioners and other providers of primary health care and social services.

Self-referral:

You can make direct referrals by calling 1800 862 363 during normal business hours at the SEPHN Mental Health Intake line.

Providers of primary health and social services can refer patients:

  • Please download and complete the MHICC referral form
  • Referrals and GP Mental Health Treatment Plans can be faxed by professionals to 1300 354 053
